Description of patient's socioeconomical status

4 Step Application
Step One
Medical provider will explain the service needed and how that service may improve the health and quality of life for their patient.
Step Two
CFSNM will review application and socioeconomic status of the applicant to verify applicant meets criteria for the fund.
Step Three
Application will be presented to Board Chairman/Advisors for approval.
Step Four
CFSNM will contact medical provider to notify them of approval and follow up on delivery of service and/or supplies.
